MW4 Campaign Overview
The
Call of Duty: Modern Warfare 4 Campaign is a single-player story led by Infinity Ward. North Korea launches a full-scale invasion of South Korea, forcing a young soldier named Private Park into battle. Captain John Price follows a separate, covert storyline that becomes part of the wider crisis.
The Campaign moves between trench warfare, large military assaults, close-quarters fighting, and special operations.
There are 16 story missions and 16 Campaign progression rewards, including eight Base Operators.

All 16 MW4 Campaign Missions in Order
Modern Warfare 4 has 16 Campaign missions, starting with Mayhem and ending with Greater Good. Each chapter earns one progression reward, including an Operator, an XP Token, or a cosmetic. The order below combines the Campaign chapter list with Activision's published reward sequence.
Complete MW4 Campaign Mission List and Rewards
The list also shows exactly which mission unlocks each of the eight Base Operators. You do not need to play on Veteran or Realism to earn these standard Campaign rewards.
| # | Campaign Mission | Completion Reward |
| 1 | Mayhem | Cho Base Operator |
| 2 | Out in the Cold | No More Rules Calling Card |
| 3 | Following Orders | Double XP Token |
| 4 | Hired Gun | Dallas Base Operator |
| 5 | Losing Ground | Ready Line Loading Screen |
| 6 | Traffic | Luca Base Operator |
| 7 | Gone Dark | Gone Dark Emblem |
| 8 | Assassins | Oh-Ji Base Operator |
| 9 | Survivors | Double XP Token |

| 13 | Escape | Double Weapon XP Token |
| 14 | Entrenched | West Base Operator |
| 15 | Decapitation | Double XP Token |
| 16 | Greater Good | Park Base Operator |
Mission-name note: Some guides refer to Mission 11 as "Deflection". We use "Defection", as listed in the published Campaign walkthrough. Activision spells the Operator names "Cho" and "Oh-Ji" in its official reward list.
Campaign Mission Highlights
The achievement list gives a preview of several missions. Mayhem features enemy paratroopers, Hired Gun has combat on moving trains, and Losing Ground includes a mortar challenge. Following Orders involves searching for contraband and notes without harming civilians, while Stranded includes yellow supply crates and a SAM Site encounter.
Entrenched features large-scale fighting around a nuclear power station. You can also read our separate
MW4 Entrenched guide and
MW4 Stranded mission guide for more detailed coverage.
Where Does the MW4 Campaign Take Place?
The story takes players across the Korean Peninsula, New York, Paris, and Mumbai. These are major Campaign settings, even when their city names do not appear as individual mission titles. Activision has shown trench combat in Korea, close-quarters encounters in New York, a vehicle chase in Paris, and SAS night operations in Mumbai.
The Paris sequence is one of the Campaign's major action set pieces, while other missions focus on stealth, urban fighting, or conventional military battles.
How Long Is the MW4 Campaign?
The
MW4 Campaign takes about 6โ8 hours for a typical story run, depending on your pace and difficulty. A focused playthrough may take around six hours, but exploring Stranded and retrying hard encounters can take longer.
PC Gamer describes a roughly six-hour experience. For trophy hunters, a completed roadmap estimates
15 or more hours because the Realism trophy requires a second full Campaign run.
MW4 Campaign Completion Time by Playstyle
| Playstyle | Time to Allow | What Changes? |
| Story on Regular | About 6โ8 hours | Based on completed reviewer playthroughs and playstyle |
| Hardened / Veteran | Varies by skill | More difficult encounters and checkpoint retries |
| Realism | A separate full run | Unlocked by completing the story once |
| 100% Achievements | About 15+ hours | At least two full runs plus collectibles and mission challenges |
If you only want to experience the story, plan for roughly one long evening or a couple of shorter sessions. Completionists should plan at least two runs because Realism unlocks only after an initial completion. All 18 trophies can be earned without completing the even harder YOLO mode.
All Six MW4 Campaign Difficulty Options
The MW4 Campaign offers four starting difficulties:
Recruit, Regular, Hardened, and Veteran. Two additional modes,
Realism and YOLO, are unlocked through Campaign progression. YOLO is an optional one-life challenge built for players who have already mastered Realism.
The published achievements specifically require Hardened, Veteran, and Realism for certain challenges. YOLO appears as an additional optional challenge, but there is no separate YOLO completion achievement in the published 18-achievement list.
| Difficulty | Challenge | Availability and Best Use |
| Recruit | Very Easy | Available from the start; easiest way to experience the story |
| Regular | Normal | Available from the start; recommended for a first playthrough |
| Hardened | Hard | Available from the start; needed for selected achievements |
| Veteran | Very Hard | Available from the start; needed for selected achievements |
| Realism | High-difficulty completion challenge | Unlocked after finishing the Campaign once; required for "Was it worth it?" |
| YOLO | One-life Campaign challenge | Unlocked after Realism completion; a death restarts the Campaign |
These challenge labels describe how each mode plays, not exact enemy-damage multipliers.
Realism keeps the punishing combat of Veteran while removing much of the HUD and enabling friendly fire. YOLO adds the risk of losing the entire run after one death.
How Do You Unlock Realism Difficulty?
Finish the complete Campaign once on any available difficulty. Realism is locked on a fresh playthrough. After completing the story, return to the Campaign options to select Realism for your next run.
The achievement
"Was it worth it?" requires completing all 16 missions on Realism. Plan a second full playthrough; Veteran alone will not unlock it. Watch your shots near allies because friendly fire is active in Realism.
How Do You Unlock YOLO Difficulty in MW4?
Complete the Campaign on Realism to unlock YOLO. Because Realism itself unlocks after your first story completion, YOLO is a further challenge after two full runs. None of the 18 Campaign achievements requires a YOLO completion.
How Does YOLO Difficulty Work?
In YOLO, you have
one life for the entire 16-mission Campaign. If you die, your run restarts at
Mayhem, the first mission. Unlike regular Realism, you cannot rely on checkpoints to recover from death.
Start YOLO from
New Game and play through the entire story. It is not a difficulty you can select for a single completed chapter from Mission Select. Practice harder missions on Realism before attempting a full YOLO run.
YOLO is separate from the Realism achievement and is meant for players who want a tougher self-contained completion challenge.
Can You Change Difficulty During the Campaign?
Completed missions can be replayed from Mission Select on a chosen difficulty. You can also adjust the difficulty during certain missions before a challenge, which may help with Hardened or Veteran achievements. The required difficulty must be active when you complete the challenge.
For the full-Campaign Realism achievement, complete all chapters on Realism rather than relying on isolated challenge attempts. Individual trophies such as Safety's On and Can't touch this can be cleaned up separately.

The reward order follows the Campaign mission sequence. Check
Activision's official Campaign rewards breakdown for the publisher's full list.
Campaign Rewards vs Achievements
Completing story missions earns the rewards shown above. Achievements and trophies are a separate set of challenges, including finishing the full Campaign on Realism and completing special tasks inside individual missions.
An achievement does not automatically grant another Operator or Blueprint. For 100% completion, track both the sixteen Campaign rewards and the separate eighteen-achievement list.
Campaign Rewards vs Digital Pre-order Bonuses
The Inner Circuit M4 Weapon Blueprint and Hunter Killer Operator Skin come from qualifying digital pre-orders, not Campaign mission completion. For the Blueprint details, read our
MW4 Inner Circuit M4 guide.

Which MW4 Achievements Are the
Hardest?
Realism is the highest difficulty required for all 18 achievements.
YOLO is harder, but it is not needed for 100% trophies. A completed trophy roadmap estimates a minimum of two playthroughs and roughly 15 or more hours. Other difficult objectives include defeating Makarov without taking damage on Veteran or higher and clearing the kitchen without shooting on Hardened or higher.
Assassins has two incompatible objectives: Revenge requires killing all remaining enemies after Shin dies, while Pacifist allows only one kill.
You must complete these in separate attempts. The three locked doors in Assassins use codes 5523, 7632, and 9103, which can make your Pacifist route much easier.

Does MW4 Have a Platinum Trophy?
There is no separate MW4 Platinum trophy on PS5. Its 18 Campaign trophies sit within the wider Call of Duty trophy structure. Xbox players can earn 1,000 Gamerscore from the same Campaign objectives.
MW4 Campaign Collectibles and 100% Completion
Several MW4 achievements require exploring beyond the main mission objective. You can collect notes, contraband, supply crates, and Dog Tags in
Following Orders, Stranded, and Escape. The locations, codes, and Base Camp repair costs below make completion easier.
| Mission | Collectible or Task | Achievement |
| Following Orders | Four notes and four contraband items; open the red toolbox with code 214 | No Stone Unturned |
| Stranded | Open all 26 yellow supply crates | Time to Crate |
| Stranded | Read 50 different notes; detailed walkthroughs identify 58 available entries | Noteworthy |
| Escape | Open the barracks-office safe with code 8410 to recover the Dog Tags | Never Forgotten |
| Base Camp | Complete 12 repairs and crafting upgrades costing a total of 195 Supplies | Scavenger |
Collectible routes identify
26 yellow crates and 58 readable notes in Stranded, plus four notes in Following Orders and two in Escape, for 64 notes in total. The
Noteworthy achievement requires 50 different notes. To be safe, aim for at least 50 in Stranded, which has enough notes on its own.
Following Orders: Four Notes, Four Contraband Items, and Code 214
In
Following Orders, find all
four notes and four contraband items for
No Stone Unturned. The contraband consists of heart medicine, an MP3 player, a Western novel, and extra rations. Search the first house fully before questioning the couple, and find the last two notes in the third house.
In the first house, the red locked toolbox uses code
214. Inside are
Note From Your Friend and heart medicine. For
No Harm Done, do not harm or threaten the civilians; use the medicine clue and peaceful dialogue options. In the third house, help the hidden woman out of the cellar before collecting
Note From Mom.
Stranded: 26 Yellow Crates and 50 Notes
Stranded is Mission 10 and features a large area that you can explore at your own pace. It contains
26 Yellow Supply Crates for Time to Crate and
58 readable notes. The easiest way to approach Noteworthy is to collect 50 notes during this single mission.
When Kal offers the choice, take the option to
find your own path and look for supplies. Check the Tac-Map and use the crate-location information you discover as you explore. Some collectible routes include hidden bunkers, locked containers, a moving train, a patrolling truck, and the SAM Site.
All Eight Stranded Container and Bunker Codes
There are
six locked shipping containers and two hidden bunkers with keypad combinations in Stranded. Enter all four digits, including leading zeroes, to open each lock.
| Container or Bunker | Code | Location Hint |
| Base Camp container | 0024 | Near the starting safehouse |
| Island Hidden Bunker | 0008 | River island bunker |
| Southeast Village container | 0078 | Village in the southeast |
| South of Jamming Station container | 8609 | South of the Jamming Station |
| Cliff near Base Camp container | 0600 | Cliffside close to Base Camp |
| Big Sign container | 1234 | Beside the large sign |
| Farm Field Hidden Bunker | 0330 | Farm fields in the northeast |
| Small Island container | 6789 | Additional river-island container |
These codes help you access locked loot locations, but they do not replace exploring the rest of the map. Some crates are inside vehicles or behind other objectives. Check the in-game crate counter before completing Stranded.
Assassins: Three Door Codes for Pacifist and Revenge
Mission 8,
Assassins, has three locked doors along its stealth route. You can enter these codes directly instead of searching for the clues while enemies patrol nearby.
| Door | Code | Location |
| Library door | 5523 | Upstairs, after the library section |
| Kitchen exit | 7632 | At the far end of the kitchen |
| Bunker entrance | 9103 | Behind the large statue in the garden |
For
Pacifist, follow the stealth route without extra kills, then enter the bunker. For
Revenge, clear every enemy after Shin dies on Hardened or higher. Before leaving the bunker area, check for the final enemies that can appear behind you.
Scavenger: All 12 Base Camp Repairs (195 Supplies)
To unlock the
Scavenger trophy in Stranded, you must complete
12 Base Camp repair and crafting prompts. The complete list costs
195 Supplies, not 80. Four basic weapon repairs cost 80 Supplies, but eight more prompts also count toward the trophy.
| Repair or Crafting Item | Cost | Important Note |
| Mortar | 30 Supplies | Check the equipment repair stations |
| Scoped LMG | 30 Supplies | Inspect the separate weapon bench |
| Vulcan Sniper Rifle Silencer | 10 Supplies | First collect the required part from the SAM Site |
| Silenced Assault Rifle | 25 Supplies | Weapon repair |
| Semtex | 5 Supplies | Equipment crafting |
| Pistol Underbarrel Shotgun | 5 Supplies | Weapon attachment repair |
| Molotov | 5 Supplies | Equipment crafting |
| Snow Ghillie Suit | 15 Supplies | Tactical gear crafting |
| Tactical Boots | 20 Supplies | Tactical gear crafting |
| Tactical Gloves | 15 Supplies | Tactical gear crafting |
| Grenade Launcher | 15 Supplies | Weapon attachment repair |
| Silenced Thermal Sniper | 20 Supplies | Weapon repair |
| Total | 195 Supplies | All 12 prompts required |
Collect Supplies from yellow crates, loot bags, and supply drops while exploring Stranded. Inspect
every bench and equipment table in Base Camp and return after visiting the SAM Site so the Vulcan silencer repair is available. Some entries are labeled as crafting instead of repairing, but they still count for Scavenger.
The workbench repairs alone are not enough. Use the
MW4 trophy roadmap as a cross-check if a repair prompt is missing, and complete all 12 before leaving the mission.
Escape: Dog Tags Safe Code 8410
The
Never Forgotten achievement is tied to the Dog Tags in
Escape. A security-office note reveals the code
8410, which opens the safe in the barracks office. Recover the Dog Tags before continuing your escape.
Can You Miss Campaign Collectibles?
Completed Campaign missions can be replayed through Mission Select, so
no Campaign trophy is permanently missable. You can return for skipped collectibles and achievement objectives after finishing the story. Check the achievement screen after each chapter.
If you are planning 100% completion, finish your first playthrough on a comfortable difficulty, collect as much as possible, then use Mission Select for unfinished challenges before the separate Realism run.
MW4 Campaign Story and Main Characters
The story takes place during a major new conflict on the Korean Peninsula. North Korea begins a full-scale invasion of the South, forcing local troops into desperate battles as the wider international crisis grows.
At the same time, Captain Price is no longer operating within the normal command structure. The two storylines bring large military battles and secret operations into the same conflict.
Private Park and the Korean War Storyline
Private Park is a young South Korean soldier
facing combat on an enormous scale. Alongside his squad and allied forces, he fights during the invasion and its aftermath.
Park's missions feature trench warfare, infantry and armored assaults, ruined urban areas, and operations deep in hostile territory. His storyline gives players a ground-level view of the conflict rather than focusing only on elite special forces.
Captain Price, Archer, and the Global Threat
Captain John Price is on the run after the events of Modern Warfare III and the assassination of General Shepherd. Task Force 141 has broken apart, and Price's former allies are not all comfortable with his methods.
Price pursues a larger threat connected to Archer and a weapon capable of changing the balance of global power. His mission unfolds away from
traditional front lines and draws him into covert operations with dangerous allies.
Valeria and the Las Almas Cartel
Price's search also brings him into an uneasy
alliance with Valeria Garza and the Las Almas cartel. This creates tension between former enemies who now have reasons to work together.
Price's separate storyline follows the events of Modern Warfare III, including the fallout from General Shepherd's death and the collapse of trust around Task Force 141. The Campaign connects those earlier events with the new international crisis.
Can You Replay MW4 Campaign Missions?
Yes. Once you complete a Campaign mission, it becomes available through
Mission Select. You can replay it to attempt missed collectibles and mission-specific achievements without starting the full story again.
Completed missions can be replayed on a chosen difficulty, including Hardened and Veteran. For example,
Safety's On requires Hardened or harder in the Out in the Cold kitchen, while
Can't touch this requires a hitless Veteran-or-higher Makarov fight in the alley of that same mission.
Keep a separate completion checklist for Realism. The achievement requires finishing the
entire Campaign on that setting, so plan a dedicated story run instead of relying on individual mission replays.
How to Replay Missions for Achievements
Open Mission Select after completing a chapter, choose the mission, and set the required difficulty for its achievement. For example, replay Out in the Cold on Hardened or higher to attempt Safety's On, or return to Escape for the Dog Tags. A full Realism completion is a separate objective.
Tips for Beating the MW4 Campaign
The best approach depends on whether you only want to enjoy the story or are trying to finish every achievement. A first playthrough should help you learn the missions before committing to harder combat challenges.
- Plan at least two playthroughs for achievements. Finish the Campaign once, then complete a dedicated Realism run. YOLO is a further optional challenge unlocked afterward.
- Use cover and reposition often. Do not stay exposed when several enemies are firing
from different directions.
- Track the collectibles. Following Orders has four notes and four contraband items. Stranded has 26 yellow crates and 58 notes, enough for Noteworthy.
- Pay attention to the exact achievement wording. Some challenges require a particular weapon, difficulty, or action before a mission objective is completed.
- Separate Revenge and Pacifist. Both are in Assassins, but you cannot earn them in a single attempt. Use door codes 5523, 7632, and 9103 to simplify the route.
- Replay individual achievement missions. Use Mission Select for Hardened and Veteran challenges, and keep the separate Realism run at its required setting.
- Complete every Base Camp upgrade. Scavenger requires 12 repair or crafting prompts and 195 Supplies. Do not stop after the first four repairs.
- Use fixed codes. Following Orders uses 214, Escape uses 8410, Assassins has three door codes, and Stranded has eight container and bunker codes.
- Prepare differently for YOLO. A single death ends the entire 16-mission run. Learn routes and difficult fights on Realism before attempting it.

If you only want the story, ignore the optional objectives on your first run. For 100% completion, use Mission Select to collect missed items and handle special challenges, then finish a dedicated Realism run. Realism also removes much of your HUD and turns on friendly fire, so watch your teammates.
